Questionnaire
One of several primary data collection techniques.

A written survey is circulated to the owner, managers, operators and/or occupants of the building to obtain information.

Questions may be either structured (to obtain yes/no answers) or unstructured (allowing the respondents to provide free-flowing commentary).


The questionnaire is submitted to a knowledge resource with sufficient advance notice prior to the commencement of site inspections.



Sample Questions

 
Electrical System
Transformers, power distribution, lighting, etc   
  1. Are there any active permit applications affecting the electrical system?    
  2. What kind of service calls relating to the electrical system have been attended to in the last 12 months?
  3. Which are currently (or have historically been) your most troublesome pieces of electrical equipment?      
  4. Are all the electrical breakers and/or fuses adequately labelled?    
  5. Have you had any problems with particular tenants/space users placing excessive demands on the circuits?  If yes, please describe:       
  6. When was the last time that the entire electrical system was shut down?        
  7. Is the exterior lighting adequate for safety and security purposes at night?   
  8.  Is the interior lighting sufficient in your facility?    
  9.  Is there any electrical equipment that is exceptionally difficult to access? If yes, please specify:
  10. What kinds of complaints do you receive from tenants &/or the general public regarding the electrical equipment               
  11. Do you have any ideas to improve energy efficiency in the building                                         
  12. Are you aware of a need for any upgrades to the electrical system?

Mechanical System
Heating, cooling, ventilation   
  1. What kinds of service calls have been attended to with the HVAC equipment in the last 6 months?
  2. Which are currently (or have historically been) your most troublesome pieces of mechanical equipment?
  3. Are there any rooms/locations in your facility that are not sufficiently warm in the winter months?    
  4. Are there any rooms/locations in your facility that are not sufficiently cool in the summer months?   
  5. Are there any rooms/locations in your facility that are not adequately ventilated?    
  6. Where do you keep the inspection logs and other reference information for the HVAC system?
  7.  Do you have all the reference information that you need for maintenance of the HVAC system?    
  8. Do you feel that the HVAC equipment’s capacity is adequate for its usage? If not, please specify     
  9. Have you received any complaints from the general public regarding the HVAC equipment? If so, what?  
  10. Are there any temporary “stop-gap” repairs to the HVAC equipment that are waiting for permanent solution?   
  11. Where do you store spare filters, belts, and other maintenance parts?

Plumbing System
Water heaters, sinks, faucets, toilets, drains, etc  
  1. Are there any active insurance claims resulting from losses such as fires and burst pipes?    
  2. Have there in been any water damage insurance claims in the past year?    
  3. Are the valves labelled in an organized way and is there a valve schedule?    
  4. Are there any valves that are exceptionally difficult to access?    
  5. When was the last time the valves were tested to determine if any were seized?       
  6. What kinds of service calls have you attended to with the plumbing equipmet in the last six months
  7. Which are currently(or have historically been) your most troublesome pieces of plumbing equipment?                 
  8. Are you aware of any current drainage problems?   
  9. Are there any temporary stop-gap repairs to the plumbing system that are waiting for permanent solution (such as gear clamps over pinholes in the piping system?
  10. What are your procedures for emergency water shutoff?        
  11. What kind of vandalism has occurred, if any, in the public washrooms                                    
  12. When was the last time the entire domestic water supply system was shutdown?       
  13. What kinds of complaints do you receive from tenants &/or the general public regarding the plumbing equipment?                     
  14. Is there any significant repair/upgrade work to the plumbing equipment scheduled to occur in the near future? If yes, please specify:
  15. Any general comments about the plumbing equipment?



Fire Safety System
Fire Detection, Fire Suppression   
  1. Have all the deficiencies from the last annual fire inspection been corrected?    
  2. Are there any outstanding orders from the fire department?    
  3. Have there been any fire damage insurance claims within the past year?    
  4. Which are currently (or have historically been) your most troublesome pieces of fire safety equipment?
  5. Are there any tenants who have repeatedly caused false alarms with the fire system? 
  6. Where do you keep the fire plan, inspection logs and other reference documents?
  7. Are there any pieces of fire safety equipment that are exceptionally difficult to access?    
  8. Do you have any recommendations for upgrades to the fire alarm system?
  9. Are you aware of any potential fire hazards?    
  10. Has there been any vandalism of the fire safety equipment during the past year?    
  11. Is there any significant repair/upgrade work to the fire safety equipment scheduled to occur in the near future? If yes, please specify:

Elevator Equipment (If applicable)

  3. Are there any floor surfaces that pose a potential safety risk (such as slippery surface when wet or trip hazards)    
  4. What kinds of vandalism and mischievious behaviour has occurred in the facility that affects the physical elements of the building – such as graffiti

Sitework System
